At the St. Gallen international expert consensus conference on the primary therapy of early breast cancer 2017, the experts voted to treat differently shrinkage mode of breast cancer with different patterns. Indeed, the current method of estimating the shrinkage mode after NAC is still under research. Here, we measured the genetic intra-tumor heterogeneity (ITH) and analyzed the MATH value of FFPE samples before and after NAC using area under the curve (AUC) of the receiver operating characteristic (ROC), and we found that Mutant allele frequency based intratumoral genetic heterogeneity (MATH) related to the tumor shrinkage mode after NAC in breast patients. 58 could be used as the MATH value threshold to distinguish the concentric shrinkage mode (CSM) and non-concentric shrinkage mode (NCSM) in NAC
